Social Care Professional

Wirral Borough Council is seeking a skilled Social Care Professional to join our Community Integrated Response Team. As a key member of our team, you will play a vital role in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers, ensuring legal framework of the Care Act 2014. The successful candidate will have a key role to play in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers, ensuring legal framework of the Care Act 2014. Wirral Council are using the 3 Conversations model (3C¿s) across all Adult Social Care teams. The ethos of the model is to identify individual's strengths and aspirations to maximise their independence and wellbeing.
Working Hours: 36 hours per week, various shift patterns (7.2 hour days) to cover 8am-8pm service, 7 days a week (weekends 9-5pm on a rota basis).
