Care Coordinator for Complex Patients

We are seeking a skilled Nurse Case Manager to join our team at Leaders In Care Recruitment Ltd. As a key member of our community healthcare team, you will be responsible for managing a caseload of clients with complex needs in the Merseyside region.
Key Responsibilities- Initial Assessments and care planning in the community
- Training and assessing support workers to ensure they have the necessary skills to provide high-quality care
- Collaborating with the multidisciplinary team (MDT) to deliver comprehensive care
- Attending meetings, making referrals, and holding team meetings to discuss progress and challenges
To be successful in this role, you will need to be a qualified Registered General Nurse (RGN) with excellent clinical skills in complex care. Your experience in managing ventilators, tracheostomy care, and peg feeds is essential.
What We Offer- A negotiable salary up to £38,000 per annum, depending on your experience
- The opportunity to earn up to £6,000 per year in tax-free bonus based on targets
- 25 days annual leave plus bank holidays and an additional day off for your birthday
- Pension and support for Continuing Professional Development (CPD) and revalidation
This is a full-time position working mainly Monday to Friday 9am-5pm with some flexibility required. The role is home-based with travel across Liverpool and Merseyside.
