Patient Care Coordinator Role: We are seeking a dedicated and organized Patient Care Coordinator to join our team in Merseyside, Cheshire, and Wirral. As a Patient Care Coordinator, you will be responsible for coordinating patient care and support services within the NHS Trust environment.


Your Key Responsibilities:

  • Personal Support: Provide essential assistance with daily activities such as bathing, dressing, grooming, using the restroom, and eating.
  • Mobility Assistance: Assist patients with their movements and positioning while adhering to NHS guidelines on manual handling.
  • Routine Management: Support patients in following established routines and sticking to their care plans.
  • Effective Communication: Engage in clear communication with patients, their families, and healthcare teams to ensure seamless care coordination.
  • Patient Transportation: Accompany patients to their appointments or organized activities within the NHS Trust as necessary.
  • Monitoring & Reporting: Observe and document any changes in patients’ health conditions or behaviors to notify healthcare professionals.
  • Record Keeping & IT Skills: Ensure precise documentation of care using NHS IT systems.

Vaccination Requirements:

In accordance with NHS standards, every candidate must have received vaccinations for certain communicable illnesses to maintain safety standards for both patients and staff. Mandatory vaccinations include Hepatitis B, MMR, Varicella (Chickenpox), BCG, and COVID-19.

Estimated Salary: £24,000 - £30,000 per annum, depending on experience.
