We are seeking a dedicated individual to join our Commissioning Team for Mental Health and Learning Disabilities. The successful candidate will play a pivotal role in ensuring the provision of integrated services or packages of care for individuals who meet continuing healthcare criteria or are subject to 117 that require commissioned services.

Job ResponsibilitiesThe Transition Case Manager will work collaboratively with other disciplines and alongside care co-ordinators to ensure individuals receive appropriate care during their transition into adult services. Key responsibilities include:
- Gathering information to understand individual needs
This is a crucial role in the pathway for those transitioning into adult services. Our team works tirelessly to ensure seamless transitions, and we need your expertise to make this happen.
RequirementsTo be successful in this role, you will need:
- Experience working in a professional capacity within the field of Mental Health or Learning Disabilities
Desirable qualifications include experience working with children and young people who have serious mental illness and/or complex needs and their families and carers in the community.
What We OfferWe offer a competitive salary of approximately £35,000 - £40,000 per annum, depending on experience.
