Dementia Specialist Care Coordinator

We are seeking a highly skilled and compassionate Dementia Specialist Care Coordinator to join our team at Southern Health NHS Foundation Trust.
Your primary responsibility will be to provide exceptional care and support to individuals living with dementia, ensuring their needs are met in a timely and effective manner.
Key Responsibilities- To conduct comprehensive assessments of individuals referred to our service, identifying their needs and risks.
- To develop and implement personalized care plans, working closely with multidisciplinary teams and service users' families.
- To deliver intensive support, including hands-on care, advice, and guidance to caregivers and community services.
- To collaborate with healthcare professionals, social care agencies, and other stakeholders to ensure seamless coordination of care.
- RMN or Allied Health Professional (Mental Health) qualification.
- Evidence-based reflective practice experience.
- Knowledge of current national legislative and professional requirements in social and healthcare.
