Job Summary:

The Pharmacy Homecare Team at Ekhuft is seeking a highly skilled and enthusiastic Pharmacist Specialist to join our team. As a key member of our team, you will play a critical role in providing pharmacy homecare services and clinical pharmacy support.

Key Responsibilities:

  • Provide pharmacy homecare services in accordance with Trust procedures and priorities set by the Advanced Level pharmacist-Homecare.
  • Apply clinical and governance arrangements as set out in the Royal Pharmaceutical Society (RPS) standards and recommendations in the Hackett Report for Homecare services.
  • Work with multidisciplinary teams to review existing and develop new homecare service programmes and specifications.
  • Collaborate with Lead Clinical Pharmacists and outpatients to identify, plan, and develop new opportunities for homecare services.
  • Identify and manage risks in relation to medicines use according to policy/protocol.

About the Role:

The Pharmacy Homecare team at Ekhuft is an intermediary between the clinical team and the Homecare company, playing a critical role in ensuring patients receive their medicines. As a Pharmacist Specialist, you will be responsible for ensuring the right drug, in the right form, is ordered and delivered at the right time to the patient.
