About the Role

We are looking for a highly organized and proactive individual to join our team as a Hospital Discharge Coordinator. As a key member of our capacity team, you will play a vital role in ensuring the efficient flow of patients through our hospital, while maintaining a focus on delivering exceptional patient care.

Your main responsibilities will include working closely with multidisciplinary teams to identify patients suitable for step-down facilities, ensuring seamless discharge planning, and maintaining close liaison with ward managers. You will also make contact with relatives/carers within 24 hours of patient admission and provide a telephone contact service.

Key Requirements:

  • Educated to 'Diploma' standard or equivalent gained through training and practical experience.
  • Flexible approach to work and willingness to work across 7 days as part of a shift rota.
  • Strong IT skills using Word, Excel, and Outlook.
