Healthcare Capacity Manager

We are seeking a skilled and dedicated individual to join our team as a NHS Bed Management Specialist. As a key member of our capacity team, you will play a crucial role in ensuring the smooth flow of patients through our hospital, providing exceptional care and meeting Trust targets.
You will work closely with multidisciplinary teams, including consultants, therapists, nurses, and social work staff, to identify patients suitable for step-down facilities and ensure seamless discharge planning. Your strong communication skills and ability to maintain close liaison within a fast-paced hospital environment will be invaluable assets to our team.

Key Responsibilities:
- Maintain close liaison with ward managers to ensure effective discharge planning of patients under their care.
- Identify patients suitable for step-down facilities in conjunction with medical and social work staff.
- Make contact with relatives/carers within 24 hours of patient admission and provide a telephone contact service.
Requirements:
- Educated to 'Diploma' standard or equivalent gained through training and practical experience.
- Flexible approach to work and willingness to work across 7 days as part of a shift rota.
- Strong IT skills using Word, Excel, and Outlook.
