About the Role
This is an exciting opportunity for a Support Practitioner to provide individualised support to adults with mental health issues, physical health & learning disabilities. The successful candidate will be responsible for helping individuals develop skills and independence, promoting their overall growth.
About You
To be successful in this role, you will have a genuine commitment to supporting people to live their best lives. You will be flexible to work on a rota and supported by an approachable management team.
Key Responsibilities:
- Providing individualised support to adults with a wide range of needs.
- Engaging with community groups and accompanying service users to attend activities they are passionate about.
- Helping individuals develop skills and independence.
