About The Role

We are seeking a motivated and compassionate individual to join our team as a Support Worker. As a key member of our support team, you will work closely with clients who have complex needs, including substance misuse, mental health issues, and challenging behaviors.

Your primary responsibility will be to provide high-quality support services to our clients, promoting their well-being and helping them to achieve their goals. This will involve working closely with other agencies, such as mental health services and substance misuse teams, to ensure a holistic approach to support.

Key Responsibilities:

  • Provide emotional support and guidance to clients, helping them to manage their complex needs.
  • Develop and implement individual support plans, working closely with clients to identify their goals and objectives.
  • Work closely with other agencies to ensure a coordinated approach to support, including mental health services and substance misuse teams.
  • Monitor and review support plans, making adjustments as necessary to ensure the client's needs are being met.

About You:

  • You will need to have a strong understanding of support needs and support planning.
  • Experience of risk assessing and risk management is essential.
  • You will be a great communicator, able to build rapport with clients and colleagues alike.
  • Accurate record keeping and computer skills are also essential.
