Community Health Care Specialist

The successful candidate will be responsible for delivering high-quality nursing care in the community, working collaboratively with the District nursing Team and other agencies to provide clinically effective care.
The postholder will be expected to assess, plan, implement, and evaluate nursing care, utilizing excellent communication skills to provide seamless patient-centred care and treatment.
Betsi Cadwaladr University Health Board delivers District Nursing Services across North Wales, working in an integrated way with partner organisations to deliver high-quality evidence-based care.
The ideal candidate will be able to travel between patients' homes in a timely manner, and possess a passion to help others.
