Job Overview

The postholder will be directly accountable to the caseload holder, responsible for assessing, planning, implementing, and evaluating nursing care in the community.

Main Duties
  1. Work collaboratively with the District nursing Team and other agencies to provide clinically effective care.
  2. Assess, plan, implement, and evaluate nursing care in the community.
  3. Travel between patients' homes, both within towns and rural areas, in a timely manner.
  4. Work in an integrated way with partner organisations to deliver high-quality evidence-based care.
