This role offers a unique opportunity to make a positive impact on the lives of others. As a Health and Wellbeing Practitioner, you will work closely with clients to develop personalized treatment plans and provide ongoing support.

The role involves managing a caseload, conducting risk assessments, and delivering structured group-work sessions. You will also be responsible for facilitating substance misuse and mental health groups, as well as providing one-to-one support.

Key Responsibilities:
  1. Develop and implement effective treatment plans that address the complex needs of clients.
  2. Foster a safe and supportive environment for clients to engage in group-work sessions.
  3. Provide expert advice and guidance on harm reduction strategies and substance misuse interventions.
Requirements:
  1. Demonstrable expertise in substance misuse and low-level mental health concerns.
  2. Proven experience in conducting risk assessments and designing SMART care plans.
  3. A deep understanding of the 12-step process of recovery from addiction.
  4. Experience in providing CBT services to individuals with substance misuse issues.
