Job Overview

The Black Country Healthcare NHS Foundation Trust is seeking a skilled Mental Health Practitioner to join their team. As a key member of the Additional Role Reimbursement Scheme, this role will involve working in Primary Care Networks to deliver high-quality Mental Health care in GP settings.

This is an exciting opportunity for Mental Health professionals from various backgrounds, including nursing, occupational therapy, psychology, and social work, to make a meaningful impact in the community.

Main Responsibilities
  1. Work as a Mental Health Practitioner in GP practices, providing initial assessments and developing treatment plans for patients with mental health needs.
  2. Complete thorough risk assessments and develop risk management plans to ensure patient safety.
  3. Make brief interventions and follow up patients who require additional support or referrals to specialist services.
  4. Support patients with health promotion and self-management of their mental well-being.
