About the Role

We are seeking a skilled Mental Health Practitioner to join our team at Black Country Healthcare NHS Foundation Trust. As a Mental Health Practitioner in Primary Care, you will be working in GP surgeries within the Primary Care Network, bridging the gap between Primary Care and Secondary Mental Health services.

Your primary responsibility will be to deliver high-quality Mental Health care in Primary Care settings, supporting individuals to access a range of interventions outside of traditional secondary care services. You will work closely with patients, completing initial assessments, identifying the most appropriate pathway/treatment plan, and making referrals or signposting to relevant services.

Main Duties and Responsibilities
  1. To work as a Mental Health Practitioner within GP practices, being the first point of contact for patients presenting with mental health problems.
  2. To complete thorough risk assessments and work with patients to develop risk management plans, ensuring the safety of patients is maintained.
  3. To make brief interventions and follow up patients that do not require talking therapies or secondary care mental health services, or patients who may require additional support to access and engage with these services.
  4. To support patients with health promotion and self-management of their mental well-being.
