The Black Country Healthcare NHS Foundation Trust is seeking a skilled Mental Health Practitioner to join their team in Primary Care. This role is part of the Additional Role Reimbursement Scheme, aiming to embed Mental Health practitioners in Primary Care Networks. As a Mental Health Practitioner, you will work closely with GP surgeries within the Primary Care Network, delivering high-quality Mental Health care to the population of the Black Country.
Main Duties- Work as a Mental Health Practitioner within GP practices, being the first point of contact for patients presenting with mental health problems.
- Complete initial assessments of patients' mental health needs and identify the most appropriate pathway/treatment plan, making referrals or signposting to relevant services.
- Conduct thorough risk assessments and work with patients to develop risk management plans, ensuring patient safety.
- Use expert knowledge to make brief interventions and follow up patients who do not require talking therapies or secondary care Mental Health services.
- Support patients with health promotion and self-management of their mental well-being.
