About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team at Black Country Healthcare NHS Foundation Trust. As a Mental Health Practitioner, you will play a vital role in delivering high-quality mental health care in primary care settings, bridging the gap between primary care and secondary mental health services.

Key Responsibilities
  1. Work as a Mental Health Practitioner within GP practices, providing initial assessments and developing treatment plans for patients with mental health needs.
  2. Complete thorough risk assessments and develop risk management plans to ensure patient safety.
  3. Make brief interventions and follow up patients who require additional support or referrals to specialist services.
  4. Support patients with health promotion and self-management of their mental well-being.
