Job Title: Care Manager for Complex Care Services

Job Summary:
Brook Street UK is seeking a skilled Care Manager to join our team in Wolverhampton. As a Care Manager for Complex Care Services, you will be responsible for assisting the Complex Care Manager with the day-to-day running of care packages, staff management, and quality assurance.

Key Responsibilities:
• Develop and implement care plans and risk assessments
• Performance management, including supervisions, return to work, sickness, and absence monitoring
• Maintaining accurate digital records and documentation
• Ensuring compliance with all regulations and standards related to care and support
• Attending regular staff meetings and training sessions

Requirements:
• Valid driving license and own vehicle
• Previous involvement in multidisciplinary teams
• Knowledge and competence in complex care needs, such as tracheostomy, ventilation, bowel, and PEG
• Minimum of Level 3 in Health and Social Care
• Excellent communication skills, both written and verbal
• Skills in assessment and care planning
• Experience working as part of a multi-disciplinary team
• Strong knowledge of care standards regulations
• Ability to work in a fast-paced environment
• Flexibility to share on-call duties
