This is an exciting opportunity to join the Respiratory Team at Imperial College Healthcare NHS Trust, a large and well-established service that provides community services in clinic and home-based settings.

The Integrated Cardio-Respiratory Services team works across hospital and community pathways, providing a range of services including diagnostic clinics, COPD and asthma disease optimisation, exacerbation management, hospital discharge, home oxygen, pulmonary rehabilitation, dyspnoea management and primary care education and support.

The role will involve facilitating and managing the provision of high-quality physiotherapy/nursing care to all adult respiratory patients, alongside consultants, AHP's and nurses in respiratory medicine, in various settings to promote respiratory health within ICHNT and the surrounding community.

Key responsibilities will include providing support and advice to doctors, nursing staff, allied healthcare specialists, patients and their families/carers, known to the community service, ensuring smooth transitions of care into and within the community setting. The respiratory practitioner will also have the opportunity to rotate through different specialisms and settings within the Integrated respiratory service.

The ideal candidate will possess highly developed specialist knowledge of their own patient group, good IT skills, and the ability to demonstrate application of the NHS Plan to their sphere of practice.

We offer a competitive salary of £50,000 - £60,000 per annum, depending on experience, as well as opportunities for career development, flexible working and wellbeing, staff recognition scheme, Cycle to Work, car lease schemes, season ticket loan or membership options for onsite leisure facilities.
