We are seeking a highly skilled and experienced Band 6 Mental Health Triage Nurse to join our team at the Kent and Medway NHS and Social Care Partnership Trust.
Key Responsibilities- Provide triage, advice, and coordination of support services to patients with mental health crises in the emergency department.
- Work autonomously with multi-agency staff to improve integration, update working practices, and promote recovery approaches.
- Conduct mental state and risk assessments of patients with complex needs.
- Develop and implement individualized care plans to ensure timely and effective support.
- Collaborate with healthcare professionals to ensure seamless transitions of care.
- Participate in quality assurance initiatives and contribute to the development of best practices.
- Registered Mental Health Nurse (RMN) with current NMC registration.
- Evidence of continuing professional and personal development.
- Degree or diploma level nursing or other relevant courses.
- Experience in Liaison Psychiatry and crisis settings.
- Audit and research experience.
- Teaching and training experience.
- Experience as a Clinical Supervisor.
