About the Role

We are seeking a highly skilled and compassionate Support Worker to join our team at Home Group Limited. As a Positive Behaviour Support Specialist, you will play a vital role in providing person-centred and strengths-based support to our customers with complex needs.

Key Responsibilities
  • Provide individualized support to customers with complex needs, including autism and learning disabilities.
  • Develop and implement personalized support plans to promote independence and social inclusion.
  • Monitor customers' mental and physical health, identifying signs of deterioration and taking proactive measures to ensure their well-being.
  • Work collaboratively with colleagues to provide a comprehensive support service.
  • Participate in ongoing training and professional development to enhance your skills and knowledge.
Requirements
  • Experience working in a complex care setting, with a Level 2 Diploma in Adult Health & Social Care or willingness to work towards it.
  • Proven ability to work with customers with complex needs, including those with challenging behaviors.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and colleagues.
  • Ability to work flexibly, including evenings and weekends, as required.
  • Enhanced DBS check and ability to maintain confidentiality.
