Disability Service Coordinator

Harrow Council's Children and Young Adults Disabilities Service 0-25 provides support to disabled young adults and their families. As a Disability Service Coordinator, you will play a vital role in ensuring that these individuals receive the necessary care and support.
Your key responsibilities will include assessing, planning, monitoring, and reviewing cases, developing and implementing support plans, and working collaboratively with children, young people, and their families.
Requirements- A detailed knowledge of the Children Act 1989 and other legislation related to children.
- Experience in casework responsibility for direct work with children and families within a statutory fieldwork setting.
- Familiarity with disabilities support and experience working with children/young people with complex health and disabilities.
- Excellent communication skills, including verbal and written communication, with the ability to use interpreters and translators when required.
- Registration with Social Work England.
This post is subject to a satisfactory enhanced DBS check. Estimated salary: £38,000 - £48,000 per annum.
