About the Role

This is a fantastic opportunity to work in a specialized role, supporting patients and their carers through high-quality assessment, planning, and care. As a Registered Mental Health Nurse (Band 6), you will be part of a multi-disciplinary team providing expert care and support to individuals with dementia and their families.

Responsibilities:
  • Assess and formulate diagnoses for patients with dementia, using your specialist knowledge and expertise.
  • Develop and implement individualized care plans, working closely with patients, carers, and other healthcare professionals.
  • Mentor and supervise staff and trainees on placement, promoting best practice and excellence in dementia care.
  • Contribute to the development and implementation of trust-wide initiatives to improve dementia care, aligned with NMC and HPC codes of conduct.
  • Collaborate with GPs, LCFT services, and non-statutory services to provide seamless care and support for patients and their carers.
Requirements:

To succeed in this role, you will need:

  • Expertise in dementia care strategies and interventions.
  • A strong ability to manage risk and adapt care plans accordingly.
  • Proven skills in multi-agency referrals and collaborative working.
  • Experience in mentoring and supervising staff or trainees.
  • Commitment to upholding trust values and delivering exceptional patient care.
