Complex Care Support Nurse

We are seeking a skilled Complex Care Support Nurse to join our team at Acuity Care Group Limited. This is an exciting opportunity for a dedicated professional to work in a field-based role, supporting complex care packages in clients' own homes across the Northwest region.

The ideal candidate will have experience working in ICU/Spinal or Complex care environments and be able to manage complex cases and provide specialized care. They will also possess strong organizational skills, a valid UK driving license, and access to a reliable vehicle.

This is a fantastic opportunity to develop your career in pediatric healthcare, working collaboratively with Complex Care Managers (CCMs), ICBs, and other professional bodies. As a Complex Care Support Nurse, you will be responsible for supporting the safe transition of complex packages from hospital to home, maintaining high standards of care for existing clients, and developing and maintaining monthly reviews with ICBs.

Estimated Salary: £42,000 - £52,000 per annum, depending on experience.
