Care Coordinator Assistant

About the Role:
We are seeking a highly skilled and compassionate Care Coordinator Assistant to join our team at Sanctuary Group. In this role, you will be responsible for providing exceptional support to our customers, ensuring their needs are met, and their lives are enriched.
Job Summary:
The successful candidate will have previous experience working with vulnerable adults or young people, particularly those with mental health needs or substance misuse issues. You will be able to work effectively under challenging circumstances, using your excellent communication skills to build strong relationships with customers, staff, and stakeholders.
Main Responsibilities:
- Develop and maintain effective relationships with customers, promoting independence and empowering them to make informed decisions.
- Provide timely and appropriate support to customers, addressing their needs and concerns in a professional and empathetic manner.
- Collaborate with colleagues to ensure seamless service delivery, sharing knowledge and expertise to enhance the overall customer experience.
- Contribute to the development and implementation of care plans, working closely with customers, families, and other professionals to achieve positive outcomes.
- Participate in ongoing training and professional development, staying up-to-date with best practices and industry developments to enhance your skills and expertise.
Requirements:
- Experience working with vulnerable adults or young people, preferably in a residential setting.
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with customers and colleagues.
- Able to work independently and as part of a team, prioritizing tasks and managing competing demands.
- Strong problem-solving skills, with the ability to think critically and creatively in complex situations.
- Ability to maintain confidentiality and handle sensitive information discreetly.
