Duty and Assessment Social Care Professional
About the Role:We are seeking a highly skilled Duty and Assessment Social Care Professional to join our team in South Gloucestershire. As a key member of our team, you will be responsible for undertaking child and family assessments, S.47 inquiries, and intervention planning for 'Children in Need'. You will use your professional knowledge to assess, monitor, and review social care needs, arranging appropriate care services where necessary.
Responsibilities:- Undertake child and family assessments, S.47 inquiries, and intervention planning for 'Children in Need'
- Assess, monitor, and review social care needs, arranging appropriate care services where necessary
- Develop individual care plans based on needs and risks, taking appropriate action to protect vulnerable people
- Manage an agreed case load, offering support and guidance, monitoring, reviewing, and reassessing care packages/services to ensure continued effectiveness
- Post-qualifying experience as a Duty and Assessment Social Worker within the UK Social Care industry
- HCPC Registration
- 1-2 years post-qualifying experience within the UK Social Care industry
- Enhanced DBS/CRB (can be obtained)
