Job Title: Social Worker Triage and Response Team

Bristol City Council is seeking skilled professionals to join their integrated teams at Southmead Hospital and Bristol Royal Infirmary Hospital.

Key Responsibilities:
  • Conducting comprehensive assessments in line with legislation, including the Care Act and mental capacity assessments.
  • Collaborating effectively with medical staff to ensure seamless patient care transitions.
Requirements:
  • A minimum of 6 months experience working in the UK as a registered social worker.
  • Possessing a relevant Social Work Qualification.
  • Registration with Social Work England.
  • Enhanced DBS, registered on the update service or be willing to apply.
  • 5 Years of referencing for agency appointments.
  • A valid driving license with access to a car.
