About the Job
  • Providing leadership and support to a diverse team of Social Workers and Assistant Care Managers within our intermediate care services.
  • Driving a dynamic approach, with a focus on developing services, embedding strength-based practice, and upskilling the workforce to deliver improved outcomes.
  • Undertaking Adult Safeguarding Enquiry Manager roles and supporting colleagues in Enquiry Officer positions to safeguard vulnerable adults.
  • Coordinating support for individuals who have maximised their independence through intermediate care provision.
  • Completing Care Act Assessments and Support Plans using a strength-based approach for individuals receiving reablement, pre-assessment, or D2A support.
  • Supporting complex Medical capacity assessments, best interest decisions, and chairing Best Interests Meetings.
  • Working collaboratively with healthcare agencies and voluntary organisations to deliver holistic care plans.
Key Qualifications and Skills
  • Social work qualification with registration with Social Work England.
  • Proven experience in managing social work caseloads and supervising others.
  • Background in community and/or group work.
  • Knowledge of children's health and social care systems, agencies, and relevant legislation.
