Job Summary

We are looking for a skilled Social Services Practitioner to join our team in Somerset. As a member of our Mental Health Inpatient Team, you will be working closely with individuals prior to discharge from hospital, providing essential support and guidance to help them achieve their goals.

As a Social Services Practitioner, you will have a strong understanding of social work principles, including independence, confidence-building, and support. You will be responsible for developing and maintaining effective relationships with service users, families, and other professionals, ensuring that their needs are met and they receive the best possible care.

The role offers a hybrid way of working, allowing you to balance office time with community-based activities. You will have opportunities to develop your skills and knowledge through training and professional development programs, including a Competency Framework linked to Career Pathways.

Our Mental Health Inpatient Team is a fast-paced and dynamic environment where you will be working alongside nationally recognized professionals who are dedicated to delivering exceptional care.
