Care Navigator Professional

We are seeking a compassionate and patient-focused individual to join our team in the NHS as a Care Navigator.
Key Responsibilities:- Support Patients Through Their Healthcare Journey: Provide guidance and direct patients to the most suitable services, ensuring they receive timely care.
- Build Relationships with Patients: Develop understanding of their needs through collaborative communication and empathetic approach.
- Collaborate with Our Team: Work alongside GPs, nurses, and allied professionals to achieve optimal patient outcomes.
- Effective Appointment Management: Coordinate appointments for smooth, efficient care delivery.
- Educational Qualification: GCSE level or equivalent.
- Active Signposting or Care Navigator Training Qualification.
- Healthcare Qualification (Level 2) or Equivalent Level Working Towards.
- Patient-Focused and Polite.
- Flexible and Cooperative.
- Motivated and Problem-Solving Oriented.
- Able to Process Information Accurately and Effectively.
- High Integrity and Loyalty Levels.
- Sensitive and Empathetic in Challenging Situations.
- Excellent Communication Skills and Ability to Understand Patient Needs.
Estimated Salary: £24,000 - £28,000 per annum. This role is part-time, approximately 16 hours per week, with alternating Friday shifts from 08:30 to 17:30 and 10:00 to 19:00.
