About the Role

We are looking for an Enhanced Clinical Professional to join our team in the Swadlincote Acute Home Visiting service.

This role will involve working as part of the wider MDT to provide proactive care planning, education, and support to patients in their own homes.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres.

Key Requirements
  • High level of clinical knowledge and understanding regarding long-term conditions agenda and wider NHS agenda
  • Management of risk and health and safety, plus safeguarding adults and children procedures and ability to implement
  • Excellent communication skills, written and verbal

The estimated salary for this role is £45,000-£60,000 per annum, depending on experience.
