Home Visiting Health Professional

The Transformation Unit is seeking an experienced Home Visiting Health Professional to join our team in Swadlincote. As a key member of our Acute Home Visiting Service, you will work collaboratively with local GP and Integrated Teams to provide support to patients in their own homes.

As a Home Visiting Health Professional, you will have the opportunity to develop your skills and expertise in areas such as long-term conditions management, risk management, and safeguarding adults and children.
Requirements- RGN or HCPC accredited degree
- Evidence of post-registration clinical professional development
- V300 Independent Non-Medical Prescriber
- Clinical Decision Making course at Level 7 or equivalent
- Teaching qualification
You will require a high level of clinical knowledge and the ability to implement proactive care planning, education, and work as part of the wider MDT.
