About the Role:

We are seeking a highly skilled and compassionate Social Prescribing Link Coordinator to join our team at Imago. As a key member of our team, you will play a vital role in supporting children and families in need of mental health services.

Key Responsibilities:

  • Work closely with GP practices to identify children and families who require mental health support
  • Deliver personalized support to patients, managing new referrals and a caseload
  • Prioritize need, recognize when to refer to specialist services, and communicate effectively across health and social care sectors
  • Establish instant rapport with children and young people, and work collaboratively with them to develop personalized support plans
  • Demonstrate a robust knowledge of confidentiality, data protection, safeguarding, and boundaries in all aspects of your work

Requirements:

  • Experience of working with children, delivering mental health support, and a background in education, medical, or social care
  • Strong organizational and communication skills, with the ability to work independently and as part of a team
  • A car driver is essential for this role, as you will be required to travel to various locations
