Care Coordinator for Adults with Learning Disabilities

We are seeking a qualified and experienced Social Worker to join our Learning Disabilities Team in Hampshire. As a Care Coordinator, you will be responsible for working with adults with learning disabilities to provide support and encouragement for independent living within the community.
Key Responsibilities- Collaborate with service users, their families, providers, and healthcare professionals to promote safe and independent living.
- Use relationship management skills to negotiate and encourage positive outcomes.
- Travel to various locations within Hampshire and the Isle of Wight or other local authorities.
- Current drivers' license and access to a reliable vehicle on a regular basis.
- Able to work flexibly and have a good balance between work and home life.

Salary and Contract DetailsThe salary range is £38,092 to £45,389 per annum (inclusive of a market supplement). The contract type is permanent, and the hours per week are 37 (part-time hours considered).
