Community Nurse Case Manager

Are you a qualified nurse with a passion for complex care? We are seeking an experienced Community Nurse Case Manager to join our client in the North West, supporting clients to live independently at home and within their communities.

About the Role

As a Community Nurse Case Manager, you will be responsible for conducting initial assessments in the community, training and assessing workers' competencies, and collaborating with the wider multidisciplinary team. You will also manage a team of non-clinical case managers and package and benefits for the role include an annual salary of £43,000, £3,000 car allowance, and mileage expenses.

Requirements

We are looking for an RGN or RSCN qualified nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management. You should have experience in tracheostomy and complex care, be a car driver, and be flexible to travel around the North West. You should also have experience caring for both adults and children.
