About the Role: We are looking for a motivated and experienced Clinical Pharmacist to join our team at North Thames Genomics Laboratory Hub. In this critical position, you will be responsible for helping the practice deliver high-quality clinical care to our patients, focusing on optimizing medicine use and improving patient outcomes. The successful candidate will have a proven track record of experience in pharmacy, excellent communication skills, and a commitment to lifelong learning.

Key Responsibilities:
  1. 1. Provide medication review services and develop pharmaceutical care plans that maximize cost-effective prescribing and improve patient care.
  1. 2. Prepare evidence-based resources and information to support the medicine management team and other relevant health professionals in implementing rational cost-effective prescribing.
  1. 3. Plan, develop, and support the introduction of new working processes within the practice to optimize the quality of prescribing.

  1. 5. Monitor patients' blood results, focusing on potential harm from long-term use of medicines or poor compliance.
  1. 6. Review patients on complex medicine regimens/with ongoing health problems and support doctors by highlighting changes in NICE guidelines.
  1. 7. Triage and manage common ailments, take part in multi-disciplinary case reviews, and deal with medication for patients recently discharged from hospital.
  1. 8. Work with the practice team to deliver repeat prescription reviews, especially for care home residents, people prescribed polypharmacy, and frail older people.
  1. 9. Look at how best practice is integrated into care processes and improve the quality of patient care, ensuring patient safety.
