Community Rehabilitation Specialist

We are seeking a dedicated Community Rehabilitation Specialist to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a key member of our community neuro rehab team, you will be responsible for delivering rehabilitation support to patients with complex neurological problems.
Your primary focus will be on providing high-quality care that promotes independent function in daily activities. You will work collaboratively with multidisciplinary teams, including occupational therapists, physiotherapists, psychologists, and speech therapists, to develop and implement personalized therapy plans.
Key responsibilities include:
- Providing visual and auditory support to assigned occupational therapists, including driving them between locations as needed;
- Delivering rehabilitation support to patients with complex neurological problems, working within a multi-disciplinary team (MDT) setting;
- Collaborating with healthcare professionals across community services, primary care, and external agencies to ensure comprehensive care;
- Utilizing technology to manage caseloads, conduct virtual consultations, and maintain accurate records;
- Assessing patient needs, planning interventions, and implementing exercise programs, adaptive equipment provision, and minor adaptations as necessary;
- Maintaining timely documentation of patient contacts, ensuring accuracy and compliance with relevant regulations;
We offer a competitive salary of £24,907 - £30,615 per annum, depending on experience. Additionally, we provide a range of benefits, including pension scheme, annual leave, and training opportunities.
Requirements
- RSA/OCR stage II - NVQ Level 3 in health and social care or equivalent experience;
- Educated to minimum GCSE level or equivalent grade A-C, including English;
- Experience of working in rehabilitation in health/social care environment at band 3 level or equivalent;
- Experience of working with people with neurological problems;
