Job Overview
Hertfordshire Community NHS Trust is seeking a motivated and enthusiastic Band 6 Community Physiotherapist to join our Integrated Care Team. This role presents an excellent opportunity for a physiotherapist to progress their career in community care.

In this position, you will work closely with the team to deliver outstanding community care, providing expert advice, guidance, and information to other professionals, carers, and relatives.

Key Responsibilities
  • Provide Physiotherapy services to patients within the Integrated ICT and Community Nursing Teams.
  • Conduct assessments and treatments for vulnerable patients with complex needs, patients in crisis, supported discharge from acute hospitals or intermediate care bed-based units.
  • Act as patient advocates throughout rehabilitation programmes, enabling empowerment through the process.
  • Utilize effective communication skills to progress rehabilitation and treatment programmes, ensuring understanding of diagnosis for self-management of conditions.
  • Address language and culture diversity issues through targeted communication strategies.
  • Assist in marketing the Integrated ICT and Community Nursing Team to develop greater understanding of service delivery within the wider population.
  • Maintain accurate and up-to-date documentation, adhering to legal requirements and communicating assessment and treatment results to relevant disciplines.

Essential Qualifications/Education
  • Physiotherapy Degree
  • Registration with Health and Care Professions Council (HCPC)
  • Post-graduate courses
  • Strong post-registration experience

Salary and Benefits
The estimated salary for this role is £43,877 - £54,656 per annum, depending on experience. We also offer a range of benefits, including flexible working options, access to training and development opportunities, and a supportive working environment.

Location
This role is based at Queen Victoria Memorial Hospital - Community, School Lane, Welwyn, AL6 9PA. The successful candidate must hold a full, valid UK driving licence and have access to a car to use regularly for business purposes.
