About the Role

Hertfordshire Community NHS Trust is seeking a skilled and dedicated Band 6 Community Physiotherapist to join our team. This role involves working as an autonomous practitioner, providing assessment and treatment for patients with complex needs, including those in crisis or supported discharge from acute hospitals or intermediate care bed-based units.

Responsibilities and Requirements
  • Provide physiotherapy services to patients within the Integrated ICT and Community Nursing Teams.
  • Conduct assessments and develop treatment plans for patients with complex needs, prioritizing their empowerment throughout the rehabilitation process.
  • Communicate effectively with patients, carers, and other professionals, addressing language and culture diversity issues where necessary.
  • Maintain accurate and up-to-date documentation, adhering to legal requirements and communicating assessment and treatment results to relevant disciplines.
Qualifications and Experience
  • A Physiotherapy Degree from a recognized institution.
  • Registration with the Health and Care Professions Council (HCPC).
  • Postgraduate courses or equivalent experience.
  • Strong post-registration experience, preferably on a broad rotational scheme.
