Job Overview
We are seeking a highly motivated and experienced Community Nurse to join our team in Havering. As a Community Nurse, you will be responsible for providing holistic healthcare to patients in their own homes, as well as in residential care homes and nurse-led clinics.
Main Responsibilities
- Provide high-quality patient care, including treatment, advice, and teaching, within the professional competence of the post holder.
- Work as a member of the integrated community health care team, liaising with GPs, other professionals, and statutory and voluntary agencies.
- Deliver individualized care to patients under the supervision of the District Nurse.
- Develop and implement care plans, taking into account the patient's needs and preferences.
- Work collaboratively with other healthcare professionals to ensure seamless care delivery.
- Contribute to the development of local strategies and contribute to Service improvement and redesign.
Requirements
- NMC Registration and current NMC R/N or any other 1st level Nursing, RN12 Adult qualification.
- Experience of working with professionals from other disciplines necessary.
- Experience of working as a member of a team but also able to work autonomously.
- Experience of providing packages of care, which involve other agencies.
- Reflective Practitioner.
- Ability to demonstrate excellent communication skills, both verbal and written.
- Team player.
- Car driver with valid UK Licence and have access to a car for work purposes.
