About the Role
  • Manage a caseload of clients through their rehabilitation journey, providing expert support and guidance.
  • Lead on assessments in the home and develop personalized rehabilitation plans.
  • Signpost and advocate for clients' needs, ensuring they receive the necessary support to live a fulfilling life.
  • Manage administrative tasks, including reporting and communication with third parties.

Requirements
