Community Care Navigator

Estimated Salary: £38,890 - £43,041 per annum

We are recruiting a Community Care Navigator to join our dedicated team at the Gloucestershire Health and Care NHS Foundation Trust. As a vital member of our First Point of Contact Centre (FPCC), you will play a critical role in navigating individuals through the mental health care system, ensuring they receive the right support and resources at the right time.

In this position, you will work closely with patients, carers, GPs, and healthcare professionals to identify their needs and preferences. Your exceptional communication and interpersonal skills, combined with your knowledge of mental health services and resources, will enable you to provide personalized navigation and support.

To succeed as a Community Care Navigator, you must possess a deep understanding of mental health principles, laws, and best practices. Registration with a professional body and post-qualification experience in mental health are essential. Key Responsibilities:

Requirements:
