We are seeking a highly skilled Community Care Specialist to join our team at Gloucestershire Health & Care NHS Foundation Trust.

About the Role

This is an exciting opportunity to work as an Acute Response Practitioner, providing urgent community care services across the County.

As a Rapid Response Clinician, you will be responsible for delivering high-quality care to patients in need of rapid response services.

You will work closely with our multidisciplinary team to provide comprehensive assessments, develop personalized care plans, and facilitate early discharge from acute hospitals when possible.

The ideal candidate will have extensive post-registration clinical experience, advanced patient assessment skills, and experience in managing complex patient needs.

A competitive salary of £34,000 - £41,000 per annum, depending on experience, is available for this role.

Required Skills and Qualifications:

  • Professional Registration (e.g. NMC or HCPC)
  • Advanced Patient Assessment Qualification
  • Experience of working in acute medical settings
