We are looking for a highly motivated and empathetic Community Nurse to provide high-quality care to patients in their own homes or community settings.
This role offers a unique opportunity to work closely with patients, families, and carers to understand their individual needs and develop personalized care plans.
As a key member of our multidisciplinary team, you will be responsible for assessing patient needs, developing care plans, and implementing evidence-based interventions.
Key qualifications for this role include:
- Degree in nursing or equivalent qualification
- Registered with the NMC
- Experience working in community health care
The estimated salary for this position is £28,500 - £36,000 per annum, depending on experience.
