**Transform Lives as a Personalised Care Coordinator**
We are seeking a highly skilled and compassionate individual to join our team as a Personalised Care Coordinator in Lancashire. As a Personalised Care Coordinator, you will have the opportunity to make a lasting impact on the lives of young people with Special Educational Needs (SEN).
You will work closely with teachers, SENCOs, and other healthcare professionals to develop and implement personalised educational and health care strategies. The ideal candidate will be a caring, patient, and proactive individual with a genuine interest in supporting students with SEN. Previous experience in a healthcare or educational support role is desirable, but not essential. Relevant qualifications in health and social care or special educational needs are preferred but not required.
Key Responsibilities:- Develop and Implement Strategies: Collaborate with staff to develop and implement personalised educational and health care strategies.
- Coordinate Care: Coordinate and deliver personalised care to students with SEN, including daily living activities such as feeding, mobility, and personal care.
- Monitor Progress: Keep detailed records of students' progress and care needs, ensuring clear communication with the wider team.
