Colorectal Specialist Nurse

We are seeking an experienced and skilled Colorectal Specialist Nurse to join our team at Oxford University Hospitals NHS Foundation Trust. As a Colorectal Specialist Nurse, you will play a vital role in providing high-quality patient care and contributing to the development of our colorectal service.
Key Responsibilities:- Provide exceptional patient care, underpinned by evidence-based practice and our trust's values.
- Work in partnership with patients, families, and carers to deliver person-centred care.
- Manage, direct, and evaluate the nursing care for a defined caseload of patients.
- Communicate effectively with patients, relatives, carers, and multidisciplinary teams.
- Develop and maintain specialist knowledge of nursing practice relevant to the clinical area.
- Undertake mentorship and preceptorship roles, reflecting our trust's values and the Oxford Model for Exemplary Professional Nursing and Midwifery Practice.
- Contribute to the development of professional nursing practice in the department.
- Work proactively to ensure every hospital visit meets the needs of the patient, ensuring effective communication with the patient, family, and other agencies.
- Be a defined point of contact for patients/carers for defined episodes/pathways of care and provide clinical care, specialist nursing advice, and psychological support.
- Be responsible for the assessment, planning, implementation, and evaluation of patient care, demonstrating a collaborative approach through effective communication and decision-making between the multi-professional team, patients, and significant others.
- Work collaboratively and in partnership with other healthcare professionals, offering advice to colleagues.
- Identify and discuss patient needs with the team, patients, carers, and the multi-disciplinary team. Be able to plan own workload and those of others as required.
- Assess health and psychological needs of individual patients and provide care to meet these needs, through a process of shared decision-making.
- Communicate complex and sensitive information effectively to patients, carers, and other staff, overcoming barriers to understanding and providing support during distressing or emotional events.
