Job Summary: We are seeking an experienced Adult Services Coordinator to join our team on a locum basis. The successful candidate will be responsible for coordinating and delivering integrated care services to vulnerable adults in need. Key Responsibilities:

  • Conduct comprehensive assessments of individuals' needs, considering physical, emotional, and environmental factors.
  • Develop and implement personalized care plans tailored to every individual's circumstances, promoting independence and well-being.
  • Provide advice, guidance, and practical support to individuals and their families/carers regarding available services and resources.
  • Liaise with healthcare professionals, community agencies, and other stakeholders to coordinate and deliver integrated care services.
  • Safeguard vulnerable adults by identifying and responding to instances of abuse, neglect, or exploitation, following established protocols and procedures.
  • Advocate for clients' rights and interests within the social care system, ensuring they receive appropriate support and access to necessary resources.
  • Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with regulatory requirements.

Requirements:

  • Relevant qualifications in social work or a related field.
  • Proven experience in adult social care, preferably in a leadership or management role.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
  • Strong analytical and problem-solving skills.
