Occupational Therapist Assistant Adults Social Care Specialist

As a key member of our multidisciplinary team, you will play a vital role in supporting vulnerable adults with physical disabilities, sensory impairments, mental health issues, and learning disabilities. Your expertise will be essential in assessing individuals, planning and implementing reablement programs, and providing equipment and adaptations to enhance their independence.
Key Responsibilities:- Manage an independent caseload, assessing individuals with complex needs and developing personalized support plans.
- Take specific responsibility for the provision of equipment, minor and major adaptations, and assistive technology to support individuals in their daily lives.
- Able to demonstrate a deep understanding of the role of an occupational therapist in a community setting.
- Able to communicate effectively with citizens, their families, and carers, and work collaboratively with a multi-skilled team.
- Basic IT skills and the ability to record work in a timely manner, prepare reports, and maintain clear case notes.
- A commitment to learning and development, with a focus on health and safety issues and the needs of vulnerable adults.
