About the Role:
We are seeking an experienced Advanced Pharmacist to join our team at University Hospitals Plymouth NHS Trust. As an Advanced Pharmacist, you will be responsible for providing high-quality patient-focused clinical pharmacy services to patients within the Healthcare of the Elderly (HCE) and Frailty areas.

Your Key Responsibilities:
- Provide specialist clinical pharmacy care to patients in the HCE/Frailty areas and related units.
- Work as a member of the Clinical Pharmacy Team, HCE, and Frailty Service Lines and wider UHP pharmacy team.
- Participate in the delivery of a high-quality clinical pharmacy service ensuring the safe, effective, and economical use of medicines for patients attending and admitted to University Hospitals Plymouth.
- Undertake audits and engage with Quality Improvement and HCE and Frailty orientated research.
- Participate in the on-call and out of hours service as per the departmental rota.
Salary: £51,944 - £72,090 per annum
