We are seeking a qualified Occupational Therapist to join our Reablement team. As a key member of the team, you will be responsible for leading holistic assessments to address reablement needs and planning, implementing and reviewing interventions. You will provide support across our multi-disciplinary team and have the opportunity to support with service development and innovation.
Reablement is a strengths-based short-term assessment and support service, aiming to help patients maintain or regain as much independence in activities of daily living. Our team is made up of Occupational Therapists, practitioners, carers, sensory staff, and business support, supported by operational management. We value teamwork and collaboration to provide the best level of care to our patients.
We are passionate about working in collaborative partnership with organisations across Health and Social care to maximise independence, prevent hospital admissions, and facilitate rapid discharge from hospital to improve outcomes for the people of Hampshire. Travel Requirements:
You will be required to travel to various locations within Hampshire, and it is essential that you have a current drivers' licence and access to a reliable vehicle on a regular basis.
